Home and Community Based Services

The purpose of Home and Community Based services is to help older adults and people with disabilities live in their home or community longer with services and support. Some of these services include adult foster care, emergency response system, homemaker services, non-medical transportation, personal care, home delivered meals, respite care, and family home care. An evaluation is completed by a case manager, through the county to determine eligibility by scoring the individual’s ability to complete self- care activities. If the individual meets eligibility, the case manager will complete a care plan, monitor, and refer to an enrolled Qualified Service Provider (QSP) to provide the services. A QSP is an individual or agency who has met the minimum standards set by the North Dakota Department of Human Services in order to be enrolled as a QSP.

In home and community services can be delivered through several programs if certain functional and financial criteria are met. Programs include:

Expanded Service Payments for the Elderly and Disabled - To qualify for this program a potential client must be Medicaid Eligible and must be receiving Supplemental Security Income (SSI), or if they are not receiving SSI, their income cannot exceed an amount equal to SSI. The person must be impaired in at least three instrumental activities of daily living. 

Service Payments for the Elderly and Disabled - To qualify, the household must have financial resources less than $50,000. Payment for services is also determined by a sliding fee scale based on the household’s monthly income. The client must also be impaired in four activities of daily living or at least 5 Instrumental activities of daily living that equal a point value depending if they live with someone or live alone. 

Medicaid State Plan - To qualify, the person must need at least one personal care service and be eligible for or receiving Medicaid.

Medicaid Waiver for Home and Community Based Services - To qualify the potential client must meet level of care criteria and be eligible for or receiving Medicaid.
