The Eligibility Unit administers four federally funded and two county-funded programs. These assistance programs help qualified individuals meet their basic needs. General Information needed

• Proof of ResidencyLease, utility bills or other documents which lists your name and current address, driver’s license with address
• Proof of citizenshipPassports, birth certificates, naturalization papers, immigration or tribal documents. Any photo ID, driver’s license
• Social Security numberActual card or receipt of application for a number or replacement card
• Household compositionA list of everyone living with you
• Verification of IncomeMost current income from last month and up to date of application. Gross payments prior to any deductions being withheld. Wages, bonuses, child or spousal support, commissions, lease or rental income, pensions or retirement benefits, self-employment, Social Security and SSI, Unemployment, VA or military benefits, Workers Comp, mineral royalties, and certain Tribal payments
• Any assetsCurrent balance of bank accounts, CDs, annuities, life insurance, policies, real estate property, stocks, bonds, retirement accounts, and trusts. List all licensed and non-licensed vehicles, including recreational vehicles and boats.
• Any expensesCurrent amounts for rents, mortgage payments, utility bills, homeowner’s insurance, property taxes, child or dependent care, court-ordered payments for child or spousal support, health insurance premiums, guardianship or rep payee fees. Medical expenses for elderly or disabled household members.

Overpayments

Information you list on the application or review must be accurate. Knowingly providing false or misleading information may result in us taking legal action against you, either criminal or civil. Your benefits may be reduced and you may be ineligible for benefits for a period of time.

Intentional Program Violations (IPV)

Intentional Program Violation will be pursued against any household, which gives false statements to mislead, misrepresent, or conceal, or withhold facts needed to accurately determine program eligibility for SNAP, Childcare Assistance and TANF. IPV penalties are 12 months for the 1st offense, 24 months for the 2nd offense and permanently for the 3rd or subsequent offense. Full and immediate repayment is requested for any LIHEAP overpayment.
